Heart Failure Clinical Nurse Specialist.

I work for TDH as the Heart Failure Clinical Nurse Specialist. This role was set up to improve the outcomes for those clients under the service which have chronic or newly diagnosed Heart Failure. The aim of this role is to decrease admissions to hospital along with decreasing length of stay in hospital by facilitating integrated health care combining care with other health care providers. This service offers community patient contact, comprehensive assessment and monitoring, liaising with existing health care providers, health education plus advocacy and support for the clients in this programme.

Referrals can be made using the Heart Failure Referral Form as below and can be done by any member of the multidisciplinary team. I will be running clinics on a weekly basis from Tui Te Ora to provide ongoing follow up and education.
