Home visits and inpatient advice/support for all manner of chronic respiratory conditions. Criteria for referral : 3 admissions over 12 months, readmission within 28 days, frequent GP visits (8 per 12 months), home O2, over 16, not pre-existing Palliative pt.

Also available to advise re home CPAP pts and new referrals for suspected Obstructive Sleep Apnoea.

Referrals accepted from GP�s, Physicians, nursing staff and other allied health professionals.
