Home visits and inpatient advice/support for all manner of chronic respiratory conditions. Criteria for referral : 3 admissions over 12 months, readmission within 28 days, frequent GP visits (8 per 12 months), home O2, over 16, not pre-existing Palliative pt.

Also available to advise re home CPAP pts and new referrals for suspected Obstructive Sleep Apnoea.

Referrals accepted from GP’s, Physicians, nursing staff and other allied health professionals.
